- Frank A. Martin - Chairman
Frank A. Martin serves as Chairman of I-trax, Inc. (Amex: DMX), the parent company of CHD Meridian Healthcare, LLC. He is actively engaged in CHDM’s strategic business development, shareholder relations and key client relationships. Previously, Mr. Martin served as I-trax’s Chief Executive Officer and President. Mr. Martin is a founder and Managing Director of the Nantucket Group, LLC, an early stage healthcare venture capital firm. Prior to the Nantucket Group, Mr. Martin founded, built and sold two healthcare companies, one in the long-term care industry, the other a healthcare information technology company that developed pharmaceutical software for physicians’ offices, Physician Dispensing Systems, Inc. (PDS). Mr. Martin sold PDS to Allscripts, Inc. in December 1996, and joined its Board of Directors where he remained until 1998. Following the sale of PDS, Mr. Martin was CEO of EduNeering, Inc., a Nantucket portfolio and educational technology company serving the pharmaceutical and managed care industries, from April 1999 to April 2000, at which point he sold the company. Mr. Martin was also in the investment banking industry for a number of years prior to starting PDS.
- R. Dixon Thayer - Chief Executive Officer
R. Dixon Thayer is CHDM’s Chief Executive Officer and has served on the Board of Directors since April 2003. In his role as CEO, Mr. Thayer provides direction and leadership toward the achievement of CHDM’s mission, strategy, and annual goals and objectives. Mr. Thayer has more than 25 years of experience in direct leadership of line management. Prior to joining CHDM, he founded and served as senior partner of ab3 Resources, Inc., a strategic consulting and private equity investment company. In addition, Mr. Thayer completed the leveraged buyout of Ford Motor Company’s GreenLeaf Auto Recycling division and serves on its Board. From 1992-2002, Mr. Thayer held instrumental positions with Ford Motor Company, Sunbeam International Division, Kimberly Clark and AFH Europe -- Scott Paper Company. In addition, Mr. Thayer served as President and Chief Executive Officer of Provant Consulting Companies, where he helped lead the merger and integration of several independent consultancies and training companies into the largest publicly traded company of its type.
- Dr. Raymond Fabius - President and Chief Medical Officer
Raymond J. Fabius, M.D., CPE, FACPE is CHDM’s President and Chief Medical Officer. He oversees CHDM’s clinical community, comprised of 1,700 physicians, physician specialists, pharmacists, nurses, and other allied health professionals. Dr. Fabius is responsible for the company’s clinical protocols and practice guidelines. Prior to joining CHDM, Dr. Fabius served as Global Medical Lead for General Electric, overseeing an ambulatory network of over 200 on-site clinics in 29 countries. Dr. Fabius served in a number of key positions with leading healthcare companies such as Aetna and CIGNA Health Plan. While with Aetna, he acted as senior medical director for Aetna’s website and data warehouse and as corporate medical director for utilization management, disease management and quality improvement. Previous to these positions, Dr. Fabius was president of an eight-doctor private practice caring for 16,000 patients. He has also held associate faculty positions at several medical schools, including Hahnemann University (now Drexel University), Children’s Hospital of Philadelphia and Thomas Jefferson University.
Board certified in pediatrics and medical management, Dr. Fabius received his medical degree from Hahnemann Medical College in Philadelphia and completed his pediatric residency training at Children's Hospital of Los Angeles. In December 2005, Dr. Fabius was elected to the Board of Directors of the American College of Physician Executives (ACPE) for a three-year term.
- Peter Hotz
- Executive Vice President, Chief Operating Officer
Peter M. Hotz is Executive Vice President and Chief Operating Officer of CHD Meridian Healthcare. He is responsible for strategic and management oversight of health center, fitness, wellness and pharmacy operations for the company’s 350+ locations. He is responsible for ensuring that all sites meet our high standards for client satisfaction, while also driving the organization to achieve financial and business objectives. Additionally, he is responsible for sales and account management for CHDM, ensuring that the sales team is focused on client-centric, comprehensive solutions from the implementation process through contract renewal and scope expansion. Mr. Hotz has developed, managed and grown several successful healthcare organizations. He joined CHDM in July 2007, as part of the company’s acquisition of Continuum, a health coaching company that he founded in 1997. Prior to founding Continuum, Mr. Hotz was the founder, president and chief executive officer of Atlantic Health Group (AHG). Under his direction, AHG grew from a start-up to $25 million in 18 months and was the leading provider of occupational health services in the eastern United States. Before founding AHG, Mr. Hotz served as Chief Operating Officer for Occupational Health Resources (OHR), and held management positions with the CareSys Company, Price Waterhouse and American International Group (AIG) in Japan. Mr. Hotz earned his B.A. degree in economics from Ursinus College and an M.B.A. from the University of Maryland.
- Bradley S. Wear - Chief Financial Officer
Bradley S. Wear is Senior Vice President and Chief Financial Officer of CHD Meridian Healthcare, joining the company in September 2007. He is responsible for all financial management of the organization and oversees the Finance, Accounting, IT and Business Intelligence departments. With more than 25 years of experience, including nearly 20 years in the healthcare and healthcare technology industry, Mr. Wear brings a unique perspective to the organization. Prior to joining CHD Meridian, Mr. Wear was the chief financial officer of Qualifacts Systems, Inc., where he was responsible for all financial aspects of the company including financial reporting, risk management, treasury, stock registry, procurement, investor and banking relations among others. Mr. Wear joined that organization in 2003. Before working at Qualifacts Systems, Wear was a founding member of digiChart, Inc., an electronic medical records company focused primarily on specialty-specific applications, where he spent nearly seven years as the chief financial officer. Mr. Wear has also held management positions within the GAMBRO Group --including positions with REN Corporation and COBE Laboratories. A graduate of Western State College, Gunnison, CO, Mr. Wear holds a bachelor’s degree in accounting and is a CPA.
- Yuri Rozenfeld - Senior Vice President, General Counsel, Secretary
Yuri Rozenfeld Yuri Rozenfeld is Senior Vice President, General Counsel and Corporate Secretary of I-trax and CHDM. He has been with the organization since July 2000. Prior to joining the company, Mr. Rozenfeld was an associate in the Business and Finance Group at Ballard Spahr Andrews & Ingersoll, LLP, where he represented small- and mid-cap public companies and venture capital funds in a broad range of corporate matters, including stock and asset acquisitions, mergers, venture capital investments, venture fund formations, partnership and limited liability company matters and securities law matters. From 1995 to April 1997, Mr. Rozenfeld was an associate specializing in product liability litigation with Riker, Danzig, Scherer, Hyland & Perretti LLP. Mr. Rozenfeld is a graduate of Amherst College and Rutgers University School of Law, where he was a member of the Law Journal. He is admitted to practice law in New Jersey, New York and Pennsylvania.
- Pam Dulaney - Senior Vice President, Operations
Pam Dulaney, RN, MA, COHN-S, CMC Pam Dulaney, RN, MA, COHN-S, CMC, is Senior Vice President, Operations for CHDM and oversees all daily operations for the company’s workplace-based health centers. She is responsible for leading and supporting management teams, medical professionals, and ancillary staff in their daily activities. She began her career with the organization in 1990 as a Wellness Coordinator. Since that time, she has served as Vice President of Operations for various divisions of the company. Ms. Dulaney has been instrumental in the development of standardized procedures for health center implementation, clinical protocols utilizing best practice standards, Continuous Clinical Quality Improvement Programs, and leadership development for our managers and the medical staff who support all of our health centers throughout the country. One of her key responsibilities is to monitor the Client Satisfaction Survey Program and the Patient Satisfaction Survey Program. CHD Meridian uses the analysis of these survey results to continue to upgrade our programs and services.
- Danny Nelms - Vice President, Human Resources
Danny Nelms is the Vice President of Human Resources for CHDM. He manages human resource practices that create a positive culture for CHDM’s 1,700 employees, ensuring the recruitment and ongoing development of a superior workforce dedicated to quality, productivity, and high standards. Mr. Nelms came to CHDM with 15 years of experience in a variety of industries including public accounting, marketing and manufacturing, with extensive experience in managing the effects of mergers, acquisitions and consolidations. Before joining CHDM, Mr. Nelms was vice president of human resources for Murray, Inc., a $500 million manufacturing company with more than 1,400 employees. Prior to Murray, he held human resource positions at Progeny Marketing Innovations, Service Experts, Inc., Robert Half International, Renaissance Hotels International and Arthur Anderson. Mr. Nelms earned his bachelor’s degree in business administration from Georgia State University in Atlanta, Georgia and his M.B.A. from The Massey School at Belmont University in Nashville, Tennessee.
